If you don't know of any contact at the charity, enter yourself as the contact in case we have additional questions about the charity or your entries. The organization or program you add does not have to be strictly a charity--any group engaged in community service or public-interest work can be listed for those activities.
You can also enter a volunteer with this same form by using "volunteer" for the charity name (please do not enter a person's name for the charity name) and selecting "Individual volunteer" for the Type of Charity. To maintain privacy, volunteers will not be visible on the list.
